MODEL OF SUPPORT :
THE UPLIFT PATHWAY
Our program uses a tiered approach to gradually decrease support as residents gain independence:
Phase 1
Intensive Support
Focus on stabilization, comprehensive needs assessment, and co-producing a dynamic Individualized Support Plan (ISP).
Phase 2
Developing Independence
Active mastery of life skills (financial literacy, household management, and digital literacy).
Phase 3
Preparing for Departure
Focus on tenancy readiness, housing applications, and sustainable exit planning.

What We Deliver
The Foundations of Independence
We deliver a holistic, three-phased "Uplift Pathway" designed to build confidence from the ground up. Our delivery model is built on three pillars:
- Specialist Clinical Insight: Expert-led support for Mental Health, Autism, and Learning Disabilities.
- Life Skills Mastery: Hands-on workshops in our teaching kitchen and resource center, covering everything from financial literacy to digital skills.
- Community Integration: Active support to re-engage with education, employment, and social networks to prevent isolation and promote long-term success.
